Details emerge in Provo Canyon School's license revocation
PCS's Springville Campus had 19 noncompliance citations in one visit
New details have emerged surrounding the Utah Department of Health and Human Services decision to revoke the operating license of Provo Canyon School’s Springville Campus on July 6, 2026.
The notice of agency action that was delivered to Provo Canyon School, which can be read in full below, outlined 19 cases of noncompliance. The notice put an immediate ban on enrollment to the school’s Springville Campus, which houses their girls’ program.
As reported earlier, all services at the campus must stop by August 6, 2026.
Noncompliance citations listed on the notice included everything from “failing to provide services and supervision that is commensurate with the behaviors and needs of each client” to “engaging in an unnecessary restraint and aggressive physical contact with a client.”
The boys’ program, housed at the Provo Campus, was the subject of a different agency action on May 19, 2026, following a May 12, 2026 incident.
